Cedar Wood Farms
Veterinary Care & Health Acknowledgement
At Cedar Wood Farms, every puppy receives routine veterinary care, vaccinations, deworming, preventative care, and daily monitoring before going home. However, puppies are living animals and illnesses can develop after they leave our care.
Veterinary Expenses:
By signing below, you acknowledge that Cedar Wood Farms is not responsible for veterinary expenses incurred after your puppy leaves our care, except where specifically stated in your purchase contract or written health guarantee.
Parasites:
Parasites including Giardia, Coccidia, roundworms, hookworms, whipworms, and tapeworms are common in puppies despite proper preventative care. We follow strict parasite prevention protocols, but no breeder or veterinarian can guarantee a puppy will remain parasite-free.
Kennel Cough:
During the hot summer months in the South, kennel cough may occasionally occur. If your puppy has been exposed or shows any symptoms before pickup, Cedar Wood Farms will fully disclose this information before your puppy goes home. You may choose to proceed with pickup or delay pickup if mutually agreed upon. As a courtesy, Cedar Wood Farms will cover the cost of the first round of antibiotics prescribed by your veterinarian for a disclosed kennel cough case. Any additional treatment, diagnostics, medications, or follow-up veterinary care remain the buyer's responsibility.
Buyer Acknowledgement:
I have read and understand this document and agree to the terms outlined above.
Please sign and return this form to Cedar Wood Farms before or at the time of puppy pickup.
